The incident reporting system is a collaborative approach between Paddle UK and the British Stand Up Paddle Association and Surfing England. The system is confidential and you do not need to be a member of any of these organisations to report an incident or safety concern.
You can use the incident reporting form to report an incident, near miss or safety concern that occurred during any paddling or surfing activity.
Reporting an incident helps us to understand the types of incidents occurring and identify trends. This helps to inform our guidance and enables us to address safety issues and concerns.
Our approach to incident reporting is based on learning from what went wrong and preventing similar incidents from happening in the future. It is not designed to point blame or result in negative consequences.
The form should be completed as soon as practical after the incident.

Data Protection
Paddle UK is a privacy conscious organisation and is strongly committed to an individual’s right to privacy. All data gathered following the completion of the incident reporting form will only be utilised to administer the incident report, determine trends, identify risks and inform future risk assessments. Any personal data provided will be processed and stored in compliance with the Paddle UK Data Protection Policy and in accordance with all applicable Data Protection laws in effect at the time of publication of this form, including but not limited to, the Data Protection Act 2018 and the UK GDPR. Data may be shared securely with statutory authorities for the purposes of investigating the incident, or with other third parties where we are legally required to do so. Where the incident report relates to, or involves, Stand Up Paddleboarding (SUP), the report will also be shared with the British Stand Up Paddle Association (BSUPA) and Surfing England as part of the coordinated approach between Paddle UK and these organisations in relation to incident management and safety within the SUP discipline. In all other circumstances, data will be anonymised before sharing.
